Add/Edit Survey

 

Add/Edit Survey in the Admin section is designed to setup questions and answers for Person Profiles, which gather person related information on a specific topic. As an example, the shelter has a questionnaire that is to be filled in when interviewing a potential new foster parent. These 2 screens (Add/Edit Survey; Edit Person - Profile tab) will help you to do it in a very efficient manner and report on the results either by person or as groups on the Reports Website.

 

First, open the Add/Edit Survey page by clicking on Add/Edit Survey from the Admin Options menu.

Then click on the Add New Survey button.  This will unlock the following required fields: Survey Name, Association & Expiry Date.

Enter the name of the survey into the Survey Name text box.  Choose the Association from the drop down list box that will be taking the surveys.  This will add the association to the person's record when the survey is taken.   If you do not want the survey to expire, enter a date very far in the future (example: 01/01/2100).  Proceed by clicking the Submit Survey button.

The Survey has now been created.  Now you may add questions to it. Click “Add New Question to Survey”. This will unlock question related fields.

 

 

Choose an existing question or opt to add questions to the drop down list for use in all surveys by typing them into the box.  Click Add.  You will receive a confirmation message and now be able to select your custom question for this survey.

Select the question from the drop down list, and mark the appropriate check boxes.  

 

Active:  Questions that currently appear on this survey.

Mandatory:  Questions that may not be skipped when filling out this survey.

Allow Multiple Answers:  Questions that may need to have two or more answers.

Order #:  Which position in the order of the questions does this question appear?

 

Click Submit Question.

Once you have submitted the Question, you can create answers for it in the next section.  Make sure your question is selected then click on Add New Answer to Question.

Select the answer from the dropdown  and click Submit Answer.

You can add as many answers as you like to each question.  

 

 

Just like you can do when creating new questions, you may create new answers for use in questions by typing them into the box and clicking Add.

Then select your answer from the drop down, mark it as Active and Submit Answer.

Once you have added all of your possible answers, you will be able to select and edit any answers.
